NHS Digital Weight Management programme - Awaiting publishing in Ardens Folder ‘Dietetics, bariatric & weight management’

Supports adults living with obesity who also have a diagnosis of diabetes, hypertension or both, to manage their weight and improve their health. It is a 12-week online behavioural and lifestyle programme.

- Referral Criteria:

- Diagnosis of diabetes (type 1 or type 2) or hypertension or both This should be the default option for this cohort of patients

- Aged 18 or over

- BMI of 30kg/m² or 27.5kg/m² BAME

- Recorded as having moderate or severe frailty

- Not pregnant

- No active eating disorder

- Not had bariatric surgery in the last two years

 

 

Specialist weight Management Service - (currently in ‘Other’ folder will move to Dietetics, bariatric & weight management)

- Tier 3 services - Pre-operative personalised programme designed to assist with weight loss and / or as a stepping stone to weight loss surgery offered at ASPH

- Tier 4 services: Wegovy (medical) & Bariatric Surgery– offered at ASPH

 

- Referral Criteria:

- Aged 18 or over

- Diagnosis of diabetes (type 1 or type 2) or hypertension or both

- BMI of 30kg/m² or 27.5kg/m² BAME

 

Oviva – NHS T2DR ( currently in Diabetes folder, move to Dietetics, bariatric & weight management)

- This programme provides a low calorie, total diet replacement treatment for people who are living with type 2 diabetes and obesity or overweight. 12 weeks TDR and 12 months 1-2-1 support

 

- Referral Criteria:

- Diagnosis of type 2 diabetes within the last 6 years, and does not use Insulin

- BMI over 27 kg/m2 or or over 25 kg/m2 BAME

- HBA1C 43-87 if on medication 48-87 if not

- Aged 18 – 65 years,

- Not pregnant, breastfeeding, eating disorder or had bariatric surgery

- Not been referred to a tier 2 weight management service in the last 6 months

- Do not have significant co-morbidity or complex needs

 

 

 

Xpert Programme Structured Education: (currently in Diabetes folder move to Dietetics, bariatric & weight management)

- Delivered over 6 weeks to increase the patients’ knowledge, skills and understanding of their condition and make lifestyle choices to manage their blood glucose levels more effectively.

 

NHS Diabetes Prevention Programme (NDPP) ( currently in Diabetes folder but move to Dietetics, bariatric & weight management)

- Provided by Living Well Taking Control – Healthier You – 9 month intervention, choice of accessing the programme in a F2F peer group setting or 1-2-1

 

- Referral criteria:

- The patient has ‘Non-Diabetic Hyperglycaemia’ (NDH)

- Age 18 or over

- Not pregnant

- No eating disorder

- HbA1c of 42-47mmol/mol or fasting plasma glucose of 5.5-6.9mmol/l; or has a history of Gestational Diabetes Mellitus (GDM) and normoglycaemia (HbA1c < 42 mmol/mol (< 6.0%) or FPG < 5.5mmol/l)

 

OYS Weight Management – (currently Other Folder move to Dietetics, bariatric & weight management)

One You Surrey Free Weight Loss Service (Gloji Digital Support/ group/1:1, sometimes offers slimming world etc)

- Referral criteria:

- NOT diagnosed diabetic or pre-diabetic

- NOT diagnosed with hypertension

- BMI >30 or 27.5 for BAME

- Not pregnant

- No eating disorder

 

Surrey exercise & weight management referral (currently in Other Folder move to Dietetics, bariatric & weight management):
